Obtaining a Patient Summary File

QuickCharts is designed to allow our customers to easily access patient reports and make fulfilling records request simple.  Besides the regular SOAP reports you are used to running, QuickCharts allows you to produce a CCD, or Patient Summary File.  QuickCharts can produce three versions of a CCD: a general Patient Info file, a Patient Referral Request and a Patient Transfer File.  This type of file can be used to transfer Patient records between other Providers\EHR systems, Health Exchanges and Portal Services.

 

The Summary Files are accessed from MU - Clinical Summary within a Patient Chart.

On the View/Export tab you will see multiple options for saving and emailing summary files.

Save (Summary) and Save (Referral) allow you to save a CCD to a user selectable location on your local computer and then use that to email/upload/deliver records via the method that was requested by your Patient.

PLEASE NOTE - Before you send a record via any method, you should have a documented set of steps you follow to verify the request is real.  You must make sure it's coming directly from the Patient or someone authorized to access the record in question.  If you are sending it electronically you must ensure the method used it Patient approved.  This might mean verifying an email address, or calling to verify Fax numbers.  These documented steps are used to help maintain HIPAA compliance. They should be part of your internal HIPAA manual and need to be reviewed and updated regularly.  

 

The Email options are used when your office subscribes to Direct Email.

Email to Patient will send a standard CCD to the secure, Direct Email address on file for the Patient.

Email allows you to enter a new, or one time use Direct Email to send a standard Summary file.

Email (Transitioned) allows you to enter a one time use Direct Email to send a Transition file.

Email (Referral) allows you to enter a one time use Direct Email address to send a Referral file.

 

When you fulfil a records request we also recommend documenting that in the Patient chart.  It should be added to the Patient Communication section of the chart and documents, like the request document itself, should be imported to the Images and Documents section of the Chart.
