Where does Box 11 come from

Insurance - Insured

Box 11 comes from the group number entered in the Policy No (Box 11) field on patient's Insurance/Insured tab.

Box 11a comes from the Birth date field on patient's Insurance/Insured tab.

Box 11b comes from the Other Claim ID (Box 11b) on patient's Insurance/Insured tab.

Box 11c comes from the Plan/Program (Box 9d/11c) on patient's Insurance/Insured tab. 

Box 11d checks YES when there is a secondary carrier.
