DENOMINATOR: The number of unique patients seen by the EP during the EHR reporting period.
NUMERATOR: The number of patients in the denominator who were provided electronic access
to patient-specific educational resources using clinically relevant information identified from
CEHRT during the EHR reporting period.
The resulting percentage must be more than 35 percent in order for a provider to
meet this measure.
All Patients with at least one visit during the reporting period count toward the Denominator.
To count for the Numerator a Patient must be provided with Patient Education material. It must be delivered via email and contain a link to an online resource.
You can add links to your system by choosing a handout from your list and right clicking the item. Add your web link in the Enter URL of Handout and press OK. Should you ever need to change the link simply right click, on the item again.

The Handout must be emailed to the Patient by pressing the Email button on the handout screen. If you have more than one Dr set up in your system you may be prompted to choose the Dr who is providing the handout

EXCLUSIONS: A provider may exclude this measure if one of the following applies:
An EP may exclude from the measure if they have no office visits during the EHR reporting
period.
Any EP that conducts 50 percent or more of his or her patient encounters in a county that
does not have 50 percent or more of its housing units with 4Mbps broadband availability
according to the latest information available from the FCC on the first day of the EHR
reporting period may exclude the measure
